The NHS and local councils are now jointly responsible for making sure that people receive the health and social care that they need, when they need it. The integration of health and social care is all about making services work better for people, centred on our needs and working in the best way to support us. If services are properly integrated, then we should be able to ask for help once, receive the support we need quickly and know what’s going on without having to be a go-between for all the different services involved in our care.
