This event hosted by the Health and Social Care Alliance Scotland and the Living Well In Communities Team at Healthcare Improvement Scotland will introduce the new national Anticipatory Care Plan (ACP) documents that were launched in June 2017. It will provide an opportunity to learn more about the impact of ACP; how to access ACP and use it in practice; and explore how ACP can benefit people living with long-term conditions.
